Dear Hearing Aid User,
DC Audiology is excited to finally offer a new battery club program. As an established hearing aid patient, you are welcome to participate in any of the programs we are now offering. The batteries are the premium Rayovac Ultra ProLine Zinc Air batteries.

Coupon Book |
Cost: $70 for size 13, 312 and 675 batteries
Cost: $80 for size 10 batteries |

| You will purchase a book of coupons (9 coupons in a book; each coupon entitles you to an 8-pack of batteries). When you need a pack of batteries, simply mail the coupon to our office and a pack of batteries will be mailed back to you. |

Stamp Card |
Cost: Free |

| With every purchase of an 8-pack of batteries, 2 stamps will be given. When 10 stamps fill your card, you are entitles to a free pack of batteries. NOTE: These batteries will not be mailed and cannot be combined with the coupon book - you must come into the office to get your card stamped. |

VIP Card |
Cost: $25 annually |

| The VIP card entitles you to 2 free in-house hearing aid cleanings as well as 6 packs of half-price batteries (8-packs). The VIP card is valid for 1 year and is non-transferable. Cannot be combined with the coupon book or stamp card options. |
In addition to the above programs, you can also special-order 48-packs of batteries. The cost of these economy-size battery packs is $40 for size 13, 312 and 675, and $45 for size 10. Unfortunately, we are unable to mail 48-pack batteries.
